No hidden fees. Just personalized care.

One-Time Enrollment Fee

$100 per family, charged at the initial visit

• Covers all family members enrolling at the same time

• If additional family members join more than 2 weeks later, a new enrollment fee will apply

• Re-enrollment Fee: $200 if more than 3 months have passed since your last active membership

  • Limited to 2 re-enrollments per patient

Monthly Membership Fee

Kids and Teens (0 to 18 years)

With an enrolled parent:

• 1 child: $45 per month

• 2 children: $40 per child per month

• 3 children: $35 per child per month

• 4 or more children: $30 per child per month

Without a parent member:

• Solo child membership: $70 per month

Students and Trainees - (Proof of enrollment required)

$45 per month
Available for:

• College students

• Graduate students

• Medical residents
  

Adults (ages 19 to 59)

$100 per month

Older Adults (ages 60 to 99)

$120 per month
Note: Due to federal regulations, we are unable to accept Medicare-covered patients at this time

Centenarians (100 and older)

$1 per month

Maternity Care

$120 per month

• Dr. Jensen provides prenatal care and delivery services for low-to-moderate risk pregnancies at Grant Medical Center

• Every pregnancy is unique. Let’s talk about what care is right for you

Billing and Payments

• Memberships are billed monthly

• Invoices are reviewed between the 1st and 4th of each month

• Payment is automatically withdrawn on the 5th

• HSA cards are accepted

Enrollment Timing:

• Join between the 1st and 15th: full month charged

• Join between the 16th and end of month: 50 percent of the monthly fee charged

Your membership begins once enrollment is accepted by our office

Cancellation:

You can cancel at any time. If you cancel before the 1st of the following month, you won’t be charged.

Labs and Prescriptions:

Membership allows access to our discounted medication and labs. Patients are responsible for lab fees and drug costs. Costs of which may vary, but are always discussed with the patient beforehand. We can utilize insurance through our vendors or utilize cash pay pricing. Which ever is the better rate.
